Your pet's documentation must be completed correctly otherwise your pet will be refused travel. Any alterations to pet documentation must be legible, signed and stamped by the vet.

Section III: Identification of animal
THE MICROCHIP NUMBER
The microchip number of the animal must be identical to the microchip number on the pet passport.
THE DATE OF MICROCHIPPING
The date of microchipping must be shown as being on or before the date of the current, valid vaccination against rabies. It must be written in the date format dd/mm/yy. A tattoo is acceptable if it was given before 3rd July 2011.
Section IV: Vaccination against rabies
All boxes must be completed:
Manufacturer, name of vaccine and the batch number. This can be a vaccine sticker
Vaccination date and expiry date
Signature and stamp of authorized vet
The voyage to the UK must take place before the 'valid until' date of the current vaccination.
The vaccination must be done after the microchipping and the animal must be a minimum of 12 weeks old before they can be vaccinated. The length of the waiting period before movement between any European country is 21 days after the first vaccination date.
A waiting period is not required for subsequent entries into the UK, provided rabies boosters are kept up to date. If the vaccination is in two parts the 21 day wait will be from the date of the second vaccination. Travel from the UK is also not permitted until 21 days have elapsed.
Section VII: Anti-echinococcus treatment
This section is only required for dogs that are arriving in the UK. The treatment must have been carried out 24 to 120 hours before arrival in the UK.
All boxes must be filled in:
Manufacturer and name of product
Date of the treatment in the upper box, in the date format dd/mm/yy
Time of the treatment in the lower box
Stamp and signature of the vet

All of this section must be completed, although telephone number is optional.
Section II: Description of animal
All of this section must be completed.
Section III: Marking of animal

Please make sure that you've booked an appointment with a vet within the 'treatment window' of 24 to 120 hours before arrival in the UK and be aware of weekend opening times and local holidays.
The Animal Health Certificate (AHC) must be in a dual language, completed by the vet in block capitals with a different colour ink OV stamp. All pages must be numbered in the format ‘Page x of y’. The AHC must be accompanied by OV certified copies of the ID and vaccination details.
Part 1 – Details
Details of the animal/s, certification of health information and vet details.
BOX I.1.
Must show all the animal owner’s details.
BOX I.2.
Must show the certificate reference number and this must be repeated on all other pages that make up the certificate. There is a box for this at the top of each standard certificate page. Any added additional pages must also include the same certificate number.
BOX I.3.
AHCs issued in the UK should be completed with DEFRA.
BOX I.4.
AHCs issued in the UK should be completed with APHA.
BOX I.5.
The same name as box I.1 should be indicated and the country of first destination in the EU. A full address is not necessary.
BOX I.18.
Should be completed with either ‘Pet dog’, ‘Pet cat’ and/or ‘Pet ferret’.
BOX I.20.
The ‘Pets’ box should have been ticked.
BOX I.28.
Must be completed fully with all requested information.
BOX II (CONTAINING II.1 – II.3 STATEMENTS)
It must have the name of territory or third country written or typed in the first statement in the box.
The issuing vet must delete all non-applicable statements for II.1. – II.3. by crossing through them.
TABLE IN II.3.
This table must show:

A waiting period is not required for subsequent entries into the UK, provided rabies boosters are kept up to date. In this case, the valid from date will be the same as the date of vaccination. If the vaccination is in two parts the 21-day wait will be from the date of the second vaccination. Travel from the UK is also not permitted until 21 days have elapsed.
As the UK has been granted Part 2 Listed status for pet travel, a blood test is not required and this box can remain blank.
TABLE IN II.4.
Required only for entry in to the UK not required for entry in to the EU. This table must show:
The microchip or tattoo number
The name and manufacturer of the anti-echinococcus treatment
The date and time of treatment and name, stamp and signature of the administering vet
OFFICIAL VET/AUTHORISED VET BOX
Must be fully completed. You must travel to the EU within 10 days of the date given by the official vet.
ENDORSEMENT BY THE COMPETENT AUTHORITY
Must be completed in full if an official vet has not completed the box above.
OFFICIAL AT TRAVELLERS’ POINT OF ENTRY
Should be completed on arrival into a EU country. This then allows use for travel around the EU and currently back to Great Britain for up to four months.
